MEMBERSHIPS
AND ASSOCIATIONS
The Neighborhood Health Services Corporation, formerly the Plainfield Health Center, is a member of then New Jersey Primary
Care Association(NJPCA) and the National Association of Community
Health Centers (NACHC).
AFFILIATIONS
CENTRAL NJ MATERNAL & CHILD HEALTH CONSORTIUM, Inc.
CNJMCHC is a regional maternal and child health planning body regulated
by New Jersey State Department of Health (NJSDOH). It recommends and approves
programs and coordinated services, regionally. NHSC actively participates
as a member of the executive board and has representation from NHSC midwifery
staff on several key committees. Several funded CNJMCHC activities are
sited at NHSC including outreach and liaison services.
MUHLENBERG REGIONAL
MEDICAL CENTER
MRMC and NHSC have administered an innovative and effective joint OB/GYN
program servicing the greater Plainfield area since 1988. This program
has provided the community with the security of board certified OB/GYN
physicians at the medical center twenty-four hours a day as well as the
expertise of a group practice model (physicians/certified nurse midwives)
at the health center.
NEW JERSEY STATE
DEPARTMENT OF HEALTH HEALTHSTART PROGRAM
NHSC has been a lauded provider of the NJDOH HealthStart program for several
years. As such, the DOH confirms that true comprehensive services, including
but not limited to case management,
outreach, and health education, are provided for prenatal patients of
the Center. This intensive and data rich service has as its key component
extensive risk assessment allowing at-risk mothers and infants to receive
early education and intervention.
CITY OF PLAINFIELD
DEPARTMENT OF HEALTH
NHSC provides the city's immunization
and sexually transmitted disease clinic services. Both of these services
are essential for maintaining short and long-term health of NHSC's maternal
and pediatric population.
UNIVERSITY OF MEDICINE
AND DENTISTRY OF NEW JERSEY-NEW JERSEY DENTAL SCHOOL
UMDNJ-NJDS has furnished an
elaborate state of the art dental operation at NHSC. In addition, the school
provides for the services of faculty level dentists and orthodontists
to provide necessary dental care for the prenatal patients at NHSC.
PLAINFIELD BOARD
OF EDUCATION
Cardinal Health Center, a satellite
center of NHSC, has recently been established at Plainfield High School.
NHSC's board certified adolescent medicine physician works at this site
to intervene early for both the prevention of at risk pregnancies as well
as assuring that pregnant teens receive care as early in their pregnancies
as possible. This is in addition to a long-standing program at the high
school whereby NHSC has provided clinical and counseling services to the
school's School Based Youth Services Program.
The Healthy Place
at Washington Community School is the newest school based initiative for
the Neighborhood Health Services Corporation. The primary goal of this center named is
to help improve access to primary healthcare services for the children
of Plainfield at the elementary school level. The mission is to improve
the overall health of our children by encouraging healthy lifestyle choices
and promoting disease prevention.
ST PETER'S MEDICAL
CENTER
SPMC provides coverage for NHSC
high-risk OB patients. The hospital also provides high level diagnostic
testing and genetic counseling.
SOLARIS HEALTH
SYSTEM
In 2002,
Muhlenberg Regional Medical Center became part of the Solaris Health System.
The Neighborhood Health Services Corporation, through its affiliation with Muhlenberg,
extended that affiliation to the Solaris Health System.
UNION COUNTY AIDS
CONSORTIUM
NHSC's participation has allowed
for access to support and referral networks for prenatal patients and
their families who are infected and affected by HIV disease. These services
can include help with food, transportation, rent, substance abuse counseling,
etc.
NJ AIDS PARTNERSHIP
Grant funding through this program
allows the addition of a full time state certified HIV counselor at NHSC.
The counselor is charged with offering HIV testing and counseling for
each prenatal patient and is available as a resource person for the OB/GYN
department staff.
