The Plainfield Health
Center has established a variety of relationships within the City of Plainfield
and beyond, to further the quality of health care to our citizens. The
following is a partial list of those affiliations:
CENTRAL NJ MATERNAL
& CHILD HEALTH CONSORTIUM, Inc.
CNJMCHC is a regional maternal
and child health planning body regulated by New Jersey State Department
of Health (NJSDOH). It recommends and approves programs and coordinated
services, regionally. PHC actively participates as a member of the executive
board and has representation from PHC midwifery staff on several key committees.
Several funded CNJMCHC activities are sited at PHC including outreach
and liaison services.
MUHLENBERG REGIONAL
MEDICAL CENTER
MRMC and PHC have administered
an innovative and effective joint OB/GYN program servicing the greater
Plainfield area since 1988. This program has provided the community with
the security of board certified OB/GYN physicians at the medical center
twenty-four hours a day as well as the expertise of a group practice model
(physicians/certified nurse midwives) at the health center.
NEW
JERSEY STATE DEPARTMENT OF HEALTH HEALTHSTART PROGRAM
PHC has been a lauded provider of the NJDOH HealthStart program for several
years. As such, the DOH confirms that true comprehensive services, including
but not limited to case management,
outreach, and health education, are provided for prenatal patients of
the Center. This intensive and data rich service has as its key component
extensive risk assessment allowing at-risk mothers and infants to receive
early education and intervention.
CITY OF PLAINFIELD
DEPARTMENT OF HEALTH
PHC provides the city's immunization
and sexually transmitted disease clinic services. Both of these services
are essential for maintaining short and long-term health of PHC's maternal
and pediatric population.
UNIVERSITY OF MEDICINE
AND DENTISTRY OF NEW JERSEY-NEW JERSEY DENTAL SCHOOL
UMDNJ-NJDS has furnished an
elaborate state of the art dental operation at PHC. In addition, the school
provides for the services of faculty level dentists and orthodontists
to provide necessary dental care for the prenatal patients at PHC.
PLAINFIELD BOARD
OF EDUCATION
Cardinal Health Center, a satellite
center of PHC, has recently been established at Plainfield High School.
PHC's board certified adolescent medicine physician works at this site
to intervene early for both the prevention of at risk pregnancies as well
as assuring that pregnant teens receive care as early in their pregnancies
as possible. This is in addition to a long-standing program at the high
school whereby PHC has provided clinical and counseling services to the
school's School Based Youth Services Program.
The Healthy Place
at Washington Community School is the newest school based initiative for
the Plainfield Health Center. The primary goal of this center named is
to help improve access to primary healthcare services for the children
of Plainfield at the elementary school level. The mission is to improve
the overall health of our children by encouraging healthy lifestyle choices
and promoting disease prevention.
ST PETER'S MEDICAL
CENTER
SPMC provides coverage for PHC
high-risk OB patients. The hospital also provides high level diagnostic
testing and genetic counseling.
SOLARIS HEALTH
SYSTEM
In 2002, Muhlenberg Regional
Medical Center became part of the Solaris Health System. The Plainfield
Health Center, through its affiliation with Muhlenberg, extended that
affiliation to the Solaris Health System.
UNION COUNTY HIV PROVIDER CONSORTIUM
PHC's participation has allowed
for access to support and referral networks for prenatal patients and
their families who are infected and affected by HIV disease. These services
can include help with food, transportation, rent, substance abuse counseling,
etc.
